What 8827 covers

• 8827(1): in-home personal care for people who need it because of age, illness, or disability — help with meals, dressing, personal hygiene, housekeeping, and companionship.
• 8827(1): Supported Living Services (SLS) for developmentally disabled individuals, and In-Home Support Services (IHSS).
• 8827(2): home care agencies that send registered nurses, licensed vocational nurses, or certified nursing assistants to deliver nursing care in patients' homes — including the non-medical help those workers give alongside it.
• Both parts include home infusion therapy when it's performed in connection with the home care or nursing care.

What doesn't belong in 8827

• Home infusion therapy performed on its own, not in connection with home care or nursing care — classified as 8852, Home Infusion Therapists.
• Private residence cleaning not performed in connection with home care — classified as 9096, Residential Cleaning Services.
• Independent Living Skills (ILS) instructional programs funded through state-contracted Regional Centers — classified as 8868.
• Work activity or supported employment programs for individuals with disabilities — classified as 8806.

• 8835 — Home, public and traveling healthcareThe counterpart outside California. NCCI-system states such as North Carolina and Massachusetts classify comparable in-home healthcare as 8835.
• 8868 — School and college professional employeesCalifornia sends state-funded Independent Living Skills instruction to 8868, not 8827.

What a premium audit looks at

• Whether caregivers hold RN, LVN, or CNA credentials and deliver nursing care — the line between 8827(1) and 8827(2).
• Which clients receive personal care only, and which receive nursing care.
• Whether infusion therapy or cleaning is performed as part of the home care or as a separate operation. California doesn't allow home infusion payroll to be divided out of 8827 unless it's a separate and distinct enterprise.
• How caregivers are engaged — as employees or as independent contractors — and the documentation supporting that status.
